The form is intended for students and families planning for college, not for children under 13 to submit information independently.
Access, providers, and your requests
Intake information is used for the advising purposes described above, not advertising. The intake workflow limits access to authorized practice administrators and uses service providers for storage, scheduling, and payment. Those providers process information under their own terms and privacy policies. You can ask about your information or request correction or deletion using the contact details in your consultation correspondence. Some records may need to be retained to provide agreed services or meet legal obligations.
Security has limits
Access restrictions help protect submitted information, but no website, email system, or online storage service can guarantee absolute security.
