What the intake asks for
The form asks for parent and student contact details, school and graduation information, optional academic and testing information, interests, support priorities, and scheduling preferences. Only share information you are comfortable providing; optional questions can be left blank.
How information is used
When submissions are enabled, Carney College Advising uses your responses to prepare for advising conversations, understand your student’s needs, and contact your family about services. The form does not sign you up for marketing.
Where information goes
Submitted intake records are stored in Supabase with access restricted to the practice’s authorized administrators. Notification systems, when enabled, receive a record identifier rather than the contents of the intake. This website does not use advertising trackers.
When you book, your scheduling information is shared with Cal.com and the connected calendar or meeting provider. When you purchase a package, Stripe processes payment details. This website does not collect or store card numbers. Each provider has its own privacy terms.
Your choices
You can review and change every response before submitting. Reloading or leaving the page clears unsent answers. For questions about an existing submission, correction, or deletion, use the counselor’s contact details in your consultation correspondence.
Before live intake opens
The practice must confirm its privacy contact and record-retention policy before accepting submissions. This notice describes the implemented data flow; it will be updated with those business details before launch.
